Eligibility Requirements/Area Served: Provides preventive dental services for
adults and children. Does not provide restorative dental procedures such as
fillings or root canals. Walk-ins will be taken on an “availability only”
basis. Scheduled appointments are preferred.

Eligibility Requirements/Area Served: Services are for clients who receive
Medicaid, Primary Care Network (PCN), or CHIP dental benefits and those
without dental insurance on a fee-for-service basis.

Fees: Accepts most Medicaid programs, Medicare, PCN (except North Temple),
CHIP, and most private insurance. Uses a sliding fee scale for uninsured patients
based on household size and income.
Eligibility Requirements/Area Served: Vulnerable and underserved individuals
in Salt Lake County regardless of their ability to pay.

market rate for self-pay only. No discounts for Faculty dentist.
Eligibility Requirements/Area Served: The uninsured or who are paying
out-of-pocket can get dental care at a discounted fee from dental students
and residents who are supervised by licensed dentists.

CHIP (Children’s Health Insurance Program)
Provides comprehensive health insurance, including limited dental, to Utah
children who qualify.

Eligibility Requirements/Area Served: U.S. citizen or legal resident, underage
19, at or below 200% of federal poverty income guidelines, and not covered by
any other health insurance.
Medicaid- Utah Department of Health
Provides health coverage for Utah adults and children who qualify. Covered
services vary according to type of Medicaid.

Eligibility Requirements/Area Served: Eligibility is determined by the
Department of Workforce Services (DWS)and varies according to type of
Medicaid. Factors such as age, income and assets, medical condition, etc. can
affect eligibility.
