services for adults and children. Does not provide restorative dental
procedures such as fillings or root canals. Majority of intakes are taken at the
beginning of each school semester.

sliding fee schedule to discount the cost of medical or dental care for those

Eligibility Requirements/Area Served: Primary dental care for the low income
and uninsured. Emergencies are accepted however, depending on funding.
New clients for ongoing treatment are not accepted. Eligible patients who
apply for this discount program must provide proof of income. Patients are
expected to pay appropriate co-pays at time of service. Applications for the
Discount Program are available at each health center. Patient may complete
an application at any time; however, the application must be approved prior
to your appointment in order to receive the discount. Patient will be
responsible for the full fees associated with the visit prior to approval.
Dental Insurance Programs
CHIP (Children’s Health Insurance Program)
Provides comprehensive health insurance, including limited dental, to Utah
children who qualify.

Eligibility Requirements/Area Served: U.S. citizen or legal resident, under age
19, at or below 200% of federal poverty income guidelines, and not covered

Medicaid- Utah Department of Health
Provides health coverage for Utah adults and children who qualify. Covered
services vary according to type of Medicaid.

Department of Workforce Services (DWS), and varies according to type of
